Social work and nursing professionals who create, monitor and manage care plans for aging loved ones, including managing health and safety and advocating with medial professionals.  Can also include companion home care services. 
Home Health Care
Supportive care delivered in your loved one’s home, including skilled care provided by licensed nurses, certified home health aides, personal care assistants or companions.
Home Renovations / Modifications / Age in Place Assistance
Modifications that allow your loved one to age in their own home with adjustments that maintain safe mobility around their living space, such as wheelchair ramps or bathroom grab bars. 
Insurance – Medicare / Medigap
Insurance professionals that will help guide you in selecting the right Medicare health insurance plans and supplements, such as Medigap plans, Medicare Part D Prescription drug plans and more.  
Long Term Care Insurance
Insurance plans which cover long term health care needs, such as care in a skilled nursing facility, assisted living community or home care. 
Managed Medicare and Medicaid Plans
